The requirement that applies today
At each CCP the organization sets up monitoring for each control measure or combination to detect any failure to stay within the critical limits, with all scheduled measurements. For each OPRP it sets up monitoring to spot a miss against the action criterion. The system records what is measured or observed and in what time frame, the method or device, the calibration or equivalent verification, the frequency, the results, who monitors and who evaluates the results. At a CCP the method and frequency must detect failure in time to isolate and evaluate product; for an OPRP they must be proportionate to the likelihood and severity. Observation-based monitoring needs instructions or specifications.

What the draft does
Subclause of 8.5.4; not listed separately in the public contents: The public contents name 8.5.4 at this position but do not list its subclauses. The subclause structure shown here comes from the held current edition; whether the draft keeps it is sold by ISO and not held here.

Why this clause matters
Monitoring is only useful if it is fast enough to hold product before it ships. A laboratory result that arrives after dispatch is verification, not monitoring. The clause asks the site to design the timing deliberately, which is exactly the question a transition auditor will ask about any new control.
Suggested evidence to discuss with your auditor
For 8.5.4.3, these are the artefacts we suggest discussing with your auditor. The standard publishes no evidence list, so this is our reading, not a requirement.
- A monitoring schedule by CCP and OPRP, with frequency, method and owner
- Calibration or verification records for the monitoring devices
- Evidence that the time frame allows product to be held before release
Where systems usually fall short
- Monitoring frequency chosen for convenience rather than control
- Observation-based checks with no written standard
How to prepare
For one CCP, trace a monitoring result to the decision it enabled. If the product had already moved, the monitoring is too slow. Redesign the frequency or the hold point.
