The requirement that applies today
The organization analyses and evaluates data from monitoring and measurement, including verification results for PRPs and the hazard control plan, internal audits and external audits. The analysis confirms that overall performance meets the planned arrangements and the system requirements, identifies the need to update or improve the system, identifies trends that suggest more potentially unsafe product or process failure, provides information for planning the internal audit programme, and demonstrates the effect of corrections and corrective actions. The results and the resulting activities are recorded, reported to top management, used in the review and used to update the system.

What the draft does
Unchanged title: The public contents keep 9.1.2 at the same number and title. No structural change is shown.

Why this clause matters
This is where data becomes a decision. The clause lists five purposes, and a report that meets none of them is waste. The trend purpose is the one that prevents incidents: a rising deviation rate on one line is a warning, not a statistic.
Suggested evidence to discuss with your auditor
For 9.1.2, these are the artefacts we suggest discussing with your auditor. The standard publishes no evidence list, so this is our reading, not a requirement.
- An analysis report that addresses the five purposes
- Trend charts with interpretation and action
- Evidence that the analysis informed the audit programme and the review
Where systems usually fall short
- A dashboard with no narrative
- Trends that were visible but not acted on
How to prepare
Take one trend and write three sentences: what it shows, what it could cause, what you will do. If the third sentence is "monitor", decide what would make you act, and by when.
